SESSION TIME-FRAME

Every session is 60 minutes.

LATENESS, CANCELLATIONS & MISSED APPOINTMENTS

Each session is one hour long. If you are later than 10 minutes of the start time, your appointment will need to be rescheduled and will be charged as a "No Show" fee.  We will not go over the allotted time period scheduled. Cancellation must be made with 24 hours notice to jonascoliver@gmail.com, for a full refund. Appointments cancelled with less than 24 hours will be charged a "No Show" fee of $75 CAN + tax.

FORMS

Clients will fill out a consent form prior to their first session that will be used for all proceeding sessions as well. Clients will also fill out an intake form that collects information about the client's present issues, needs and why they have interest in hypnotherapy. It also provides assessment as to whether the client is a good candidate for hypnotherapy or if they should be referred for medical/psychotherapeutic assistance.

Before attending your first session, please make sure both your Intake Form and Consent Form are completed. Otherwise, the session will need to be rescheduled. 

PAYMENT

Consultations are free. Each session is $110 CAN plus applicable tax. You may purchase a package of 10 sessions for $900 CAN + tax. Payment is made at the time of booking a standard session online.

Packages of sessions must be used within 6 months of purchasing.

CONFIDENTIALITY

Client information is stored on Canadian software. Sessions are confidential. Any information you choose to share with Jonas is held in strict confidence, subjection to the provisions and exceptions as set out in the Personal Health Information Protection Act (“PHIPA” or “the Act”), which is the statute applicable to health care practitioners in the province of Ontario, Canada.
